Application Process and Forms:
Local coursework applicants MUST complete:

Application for Admission into a Postgraduate Coursework Program form.
Please note section D2 of this form asks you to provide names and contacts of referees; you do not need to complete this, however you MUST provide written references (see below).

All applicants MUST provide a total of two WRITTEN references. This can be either academic or professional or a combination of both:Academic Referee Report Form

- A current CV detailing your work and research experience (if applicable).

Students who wish to be considered for a Commonwealth Supported Place (CSP) for the PGDip of Health Social Sciences, Master of Health Social Sciences and Master of Public Health MUST indicate this in Section B1 of the Application for Admission form. Students are also asked to refer to the School's Policy for CSP prior to applying.

Course Specific Requirements
The following Courses require you to submit specific documentation that MUST be included with your application in addition to completing the above. Applications will only be considered when all relevant documentation is received.
Master of Public Health:
Provide a statement of approximately 250 words summarising the reasons you wish to undertake a Masters in Public Health (DO NOT complete section B2 of the Application for Admission form instead simply attach your statement to the application form).
Biostatistics Courses:
Please include documentary evidence of the completion of a subject equivalent to a first-year undergraduate statistics subject. If you do not have undergraduate coursework in mathematics, please attach a separate sheet addressing the requirement to demonstrate a capacity for advanced mathematical work e.g. by level of achievement in high school math.
