Application Process and Forms 

Local coursework applicants MUST complete:

  1. Application for Admission into a Postgraduate Coursework Program form [PDF]

  2. All applicants must provide a total of two references.  This can be either academic or professional or a combination of both:

  3. Application for Credit and/or Exemption (if applicable to your application) [PDF]

  4. Course Specific Requirements (Master of Public Health and Biostatistics ONLY):

    The Master of Public Health and Biosatistics Courses require you to submit specific documentation/forms or evidence of completion that MUST be included with your application in addition to completing the forms above.  Applications will only be considered when all relevant documentation is received.

    Master of Public Health:

    Provide a statement of approximately 250 words summarising the reasons you wish to undertake a Masters in Public Health.  This statement is very important and, in conjunction with the rest of your application, will be used in assessing your eligibility for this Course and your ranking for a CSP Place.

    All students must complete an Application for Consideration for a Commonwealth Supported Place (CSP)  [PDF]

    Biostatistics Courses

    Please include documentary evidence of the completion of a subject equivalent to a first-year undergraduate statistics subject.  If you do not have undergraduate coursework in mathematics, please attach a separate sheet addressing the requirement to demonstrate a capacity for advanced mathematical work e.g. by level of achievement in high school math.
