Request for Amendment

Approval Process

Request for Amendments are usually approved, or recommended for approval by the Chair of the School of Population Health Human Ethics Advisory Group (SPH-HEAG) out of session and don't require review at an SPH-HEAG meeting.

Amendments to Minimal Risk and Project-within-Program
 
Once approved, a formal letter will be sent to the responsible researcher. The Request for Amendment is sent by the SPH-HEAG administrator to the Human Ethics Sub-Committee (HESC) for noting.

Amendments to Project and Program applications

Once recommended for approval the SPH-HEAG Administrator will forward the Request for Amendment to HESC. Once their review is complete you will be contacted regarding the outcome.

Submitting the Request for Amendment

  • Request for Amendments need to be submitted online via Themis 
  • Once submitted online, one hard copy of the information entered in Themis needs to be printed and submitted to the SPH-HEAG administrator.
    A copy of any supporting paperwork that has changed as a result of the request needs to be attached

Once printed the Responsible Researcher needs to sign the Request for Amendment
